Chicago Section 1201
High
School Scholarship
The ASQ Chicago Section 1201 will be awarding a scholarship to a graduating high
school student who wishes to pursue a higher education during the 2011 –2012
academic year. Scholarship applications must be postmarked by December 1, 2011;
the scholarship winner will be announced by March 1st with the award
being made in April 2012. The amount of the award is $1,000.
Eligibility and Qualifications
The scholarship is
open to any high school senior in the Chicago Metropolitan area sponsored by a
member of Chicago Section 1201. The applicant must be planning and eligible to
continue his or her education at an accredited university, college, vocational
school or junior college offering an associate or bachelors degree.
Candidates must have a cumulative grade point average of 3.0 on a
4 point scale or 3.75 on a 5 point scale, complete the scholarship application
and submit two letters of recommendation.
Students may apply for and receive a scholarship only once.
Criteria for Selection
Applicants will be
evaluated by a panel of judges on a Quality essay on “what quality means to
me”.
Application Instructions
It is the responsibility of the applicant to complete all steps
of the application procedure. No application will be considered complete until
all information requested is provided. A complete application must
contain:
The application form.
An official copy of the applicant’s academic record.
Only sealed envelopes containing the high school transcript will be accepted.
A list of academic honors, awards and activities while in
high school.
A list of community service activities, outside interests and
extracurricular activities.
Quality
essay 4- 6 pages in length.
Two letters of recommendation: one from a member of the ASQ
Chicago Section in good standing and one from a teacher, person from the
community or employer but not a relative.
Deadline
Candidates must submit the completed application and references
postmarked by December 1, 2011

Scholarship Administration
Scholarship recipients

institution for school related expenses for the 2011 – 2012 school year.

Training Institute Scholarship
Students enrolled in specified Training Institute core courses will be
automatically eligible for one free Training Institute course awarded to the top
scoring student in each of the following core courses:

·Quality Management
·Blue Print Reading
Scholarship Administration

tuition only.

The
American Society for Quality Chicago Section 1201 challenges its members to
promote quality by sharing their success stories.
Award
Description
The ASQ Chicago Section 1201 is offering a Section Member Quality Award (SMQA)
for a quality paper written by a member of the Chicago Section 1201 during the
2011 –2012 fiscal year. The paper must be written on the topic specified below
under the heading “Quality Paper”.
Applications must be postmarked by September 1, 2011 and the winner will be
announced by December 1, 2011. The amount of the award is $1,000 payable
directly to the member. The Section reserves the right to publish any
submissions with the author’s approval. The winning entry will be published in
the Section Newsletter at the discretion of the Chicago Section 1201 Board
Council.
Eligibility
The SMQA
award is open to any member of ASQ Chicago Section 1201. The member’s ASQ
membership and Section membership must be in good standing at the time of the
application. Members are eligible to receive an award once every five years.
Quality Paper
Applicants must
submit a paper on “How your dedication to quality supports ASQ’s goal to be
“The Global Voice of Quality ®”. The submission must be an original
paper.
If a paper is published,
the member can use the authorship for recertification credits and/or as part of
fulfilling a membership upgrade.
Application Instructions
Complete the application form.
Prepare a quality essay 8 - 10 pages in
length, type written and double-spaced using 12-point font size.
