1. Advance Registration is strongly advised, and payment must be received with application.

2. If you are an ASQ Member please bring your Membership card to Registration.

3. The difference between the Member and Non-Member Course Fee may be applied – through the 4th class session of each course – toward a Membership in ASQ. (Applications available on the first day of class.  Return completed applications to the Secretary/Site Manager on the first day. 

5. Course materials are included in the fee.  (Indiana Primers for all Review classes.)

6. You will receive a receipt and course materials at the first class.

7. If you register less than 5 days before the 1st class, you may have to wait until the following class for course materials. SO – Please Register Early.

8. On-Site Registration is held on the first day of classes – 8:30 AM for Saturday morning classes.  6:00 PM for weekday evening classes.

9. These courses are offered by the ASQ Chicago Section Training Institute and are not associated with credit programs at the host schools... 

10. The Training Institute reserves the right to cancel courses.  Enrollment minimum is 6 students per course or at the discretion of the Training Institute Director.

11. In-Plant Training: Companies may take advantage of any of these courses for In-Plant Training. Contact Andrew Weiss or at (773) 237-0592
